Example text

This is complex and time-consuming, and exceedingly unpredictable in terms of therapeutic outcome. Even if important therapeutic effects are observed, it is very difficult to say what factors they were due to. The final result is that the relevance of psychodynamic thinking to neurotic disturbances is by no means obvious. Dramatic examples like the Geologist, in which a single wellaimed intervention, concerned solely with the current conflict, is followed by improvements that prove to be sufficient, are the exception rather than the rule.

Moreover, although he had been married for 3 years, he had known her for 5 years, so that his relation with her more or less coincided with the duration of his symptoms. In a later part of the session he described the building up of frightening tension in his body, moving his hands upwards over his ribs. The therapist pointed out that this gesture might as easily describe the building up of anger inside him (an interpretation of the hidden feelings, here an impulse)', to which the patient said that if he expressed any anger to his wife she would leave him (a statement of the anxiety).

This is an empirical question to which there is no general answer. In the present case it is clear that the patient received considerable immediate relief from his single interview, but whether this was of any lasting value to him can only be shown by subsequent events. In fact we have a 3-year follow-up, from which the following became clear: (1) The patient's mother consistently refused to be transferred, but the patient reacted constructively to this, reaching a reasonable compromise between his needs and hers.
