But the fact that we can never observe the whole of ourselves need not prevent us from scrutinizing what we can with accuracy. I may not be able to see my own back; but I can get a good idea of it with the help of a mirror, or by comparison with someone like myself. One of the functions of the therapist is to provide that mirror, and to model the possibility of that empathic comparison. The act of verbalization makes possible critical appraisal. If one can talk about an emotion, one is, at least at that moment, no longer possessed by it.

The dream is thus a probe into the unconscious, and if patient and therapist together can put it under the analytic microscope there is invariably much to be learnt. It will point to the fundamental constellations of the patient’s psyche, as well as often unnoticed or unconscious reactions to what is currently happening. Patients generally know that therapists are interested in dreams. It is therefore unsurprising that in the course of her work, the psychotherapist is likely to be ‘offered’ a dream by the patient, for consideration and comment.

Such patients may exhibit obvious anxiety, but are more likely to be withdrawn and unapproachable. People suffering from paranoid psychosis may fear in a delusional way that the therapist can actually read or control their minds, rather than these being metaphorical, ‘as if ’ anxieties. Even the most experienced diagnostician may fail to detect psychosis in a single consultation. Although some psychotic patients need, and to some extent respond to, psychotherapy, they are best not taken on by beginners.
